本季描绘重返医院的牧野医生与升入初中的学生们的新篇章。藤野一希难以适应中学生活，父亲突然带回新母亲与妹妹更令其家庭归属感崩塌；而顺利完成心脏病手术的冴岛直明升入新年级，虽立志成为牧野医生般的医者，内心仍怀不安。此时，一名因病复发失去求生意志的少女出现在牧野面前。除原作剧情外，本季将新增原创角色故事线。
